Student Recitals
Student Recital Schedule Form and Information
Recital Program Approval Form. This form is designed to notify your supervisory committee of your recital date and program choice.
Please contact Chris Smith, Recital Scheduler to:
- Reserve Dumke Recital Hall and the Clark Green Room
- Learn the policy and procedures for printed programs
- Obtain and complete the Printed Program Request Form
- Turn in all forms (except the Course Registration Form which goes to the music office) and your deposit

Special Course Applications and Private Lessons: Forms must be completed each semester you take the course even if you are continuing with the same teacher. Application deadline for Fall 2009 is August 26, 2009. Late applications will be refused. Application forms are also available in the Music Office. Private lesson, coaching, and recital courses require an additional special fee.

All graduate students at the University of Utah are required to document progress through their degree program with a series of forms. Each form coincides with a significant milestone in the degree and must be submitted by the deadlines listed in the degree program calendar (see "Deadline Calendars" below).
