Late Applications WILL NOT Be Accepted
• Continuing Students - Turn in your application with advisor’s signature in December and receive your add number. New students can wait until after arrival in Salt Lake City.
• Come to the Music Office to turn in forms and get your add numbers.
• Register for your courses by the 4th day of classes. After that point you will need an additional permission code. You must register by January 26, 2009.
• It is solely your responsibility to register for your courses.

Research/Recital/Thesis/ Project Registration Applications
Due by 5:00 PM, Wednesday, January 14, 2009

• You must register for recital, research, thesis, or project by January 26, 2009.
• It is solely your responsibility to register for these courses.

Remember you must be concurrently registered for private lessons the
semester of your final recital/project for MM and DMA performance majors and MM composition
majors. •
Please note: to schedule your recital date, you must complete the Recital Schedule Form and the *new* Recital Program Approval Form
Final Recital: if you are registering for your final recital, please remember to schedule a date for your Oral Defense. It must take place within a week of the final recital, and cannot take place after the last day of Fall classes. Please consult your Supervisory Committee, schedule the date, and submit the Exam Defense Date Form three weeks before the defense date.
