Certification and Licensure

Although your degree is in music, ALL education majors receive certification and licensure through the University of Utah College of Education and the Utah State Office of Education. Music education majors are no longer advised by the Department of Teaching & Learning. Instead, advising matters regarding certification and licensure will go through the music education faculty in the School of Music. This information will be forwarded to the College of Education and ultimately to the Utah State Office of Education for their consideration. Instrumental music education majors should see Dr. Ely about matters concerning certification and licensure. Choral music education majors should see Dr. Napoles about matters concerning certification and licensure.

All music education students must complete the LICENSURE APPLICATION to initiate the process of becoming licensed to teach music in the state of Utah. The licensure application and instructions for completing the application are available to you from Mindy in the College of Education (Rm. 225, MBH), from Dr. Ely (Rm. 314, DGH) and in the Music Education Lab (Rm. 320, DGH). YOU NEED TO COMPLETE THIS APPLICATION IMMEDIATELY AFTER STUDENT TEACHING TO RECEIVE YOUR TEACHING LICENSE! This process cannot be completed until YOUR DEGREE HAS BEEN POSTED. Once your degree is posted, take a copy of your transcript indicating that you have in fact graduated to Dr. Ely (Instrumental Majors) or Dr. Napoles (Choral Majors). Dr. Ely or Dr. Napoles will then complete the Departmental Recommendation Form (signed by Dr. Walzel) AFTER your degree has been posted and will submit all licensure materials to Mindy in 225 MBH. Mindy will then forward these materials to the state office. MATERIALS SHOULD BE SUBMITTED TO DR. ELY (INSTRUMENTAL MAJORS) OR DR. NAPOLES (CHORAL MAJORS) WITHIN THE FIRST TWO WEEKS FOLLOWING THE END OF THE SEMESTER.

Licensure applications should contain the following:

1. Official Transcript once degree is posted (give to Dr. Ely or Dr. Napoles)
2. Application for licensure form (get from Mindy or Dr. Ely or Dr. Napoles)
3. Statistical information form (download from this Web page)
4. Money order for $50.00 payable to Utah State Office of Education (USOE)
5. Departmental recommendation letter (get from Dr. Ely or Dr. Napoles)

The licensure application fee is $50. This fee payment must be in the form of a MONEY ORDER and made payable to: Utah State Office of Education or USOE. Once you have completed the process, TURN IN ALL MATERIALS TO DR. ELY (Rm 314) OR DR. NAPOLES (Rm. 256).
